DIRECTOR OF CASE MANAGMENT

STATUS:  Full Time

  EDUCATION:  Bachelor of Science Degree in Nursing, preferred.  Current license in the State of Texas.

 REQUIREMENTS:  Responsible for the coordination and implementation of case management strategies for appropriate utilization of services and hospital admissions pursuant to the Case Management Plan.  Plans and coordinates care of the patient from pre-hospitalization through discharge and post-discharge for patients receiving care coordination.  Works with all members of the healthcare team to assure a collaborative approach is maintained in the care and treatment of the patient.  Reviews care and the treatment plan for appropriateness of admissions (IP or OBS) utilizing screening criteria.  Responsible for the authorization of appropriate patient services for inpatient or observation stay for patients. Plans and coordinates home care services and needs, to prevent unnecessary admissions.  Completes, in a timely fashion, documentation of all case management, and care coordination strategies performed into the appropriate electronic health record/database ensuring to comply with current standards and policies.                                                                                                                                  

EXPERIENCE: Minimum of 3 or more years of case management experience, preferred.  5 or more years of experience in an acute hospital, home health, hospice, inpatient mental facility.  Ability to read and communicate effectively in English.  Additional languages preferred.  Strong written and verbal skills.  Basic computer knowledge.  Current clinical knowledge.  Understanding of healthcare/HMO industry, CDI and CM principles.  Knowledge of financial and/or statistical analysis
